Online ‘Pre-Registration’ With The Practice
Online registration at Aldersbrook Medical Centre is quick and easy.
Please complete the following steps:
1. Please fill out the ‘Pre Register with Patients Access’ form – link found below and press submit.
2. Once the form has been submitted, please come into the practice during our opening hours with 1 proof of ID (e.g. passport) and 1 proof address (e.g. utility bill/bank statement) and we will come complete the registration process for you. ( Due to Covid-19 Pandemic we may ask you to email us these)
3. If you wish to speak to a doctor following registration – please let a member of staff know.
If required, all our newly registered patients are offered:
An Appointment with the GP within 48 hours

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
