We are part of Wanstead and Woodford Primary Care Network. The Primary Care Network provides a wider range of services to patients and to more easily integrate with the wider health and care system.

  1. Clinical Pharmacist.

Clinical Pharmacists work as part of the multidisciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.  They do medication reviews, medicine reconciliation and compliance checks.

  • Social Prescribing link worker

Social Prescribing Link Workers connect people to community groups and agencies for practical and emotional support and complement other approaches such as care navigation and active signposting.

  •  First Contact Physiotherapist

First Contact Physiotherapists can assess, diagnose, treat and manage musculoskeletal (MSK) problems and discharge a person without a medical referral. Patient can directly book appointment with them.

  • Community Paramedics

Paramedics from London Ambulance based at practice and carryout home visit for housebound patients in Wanstead and Woodford PCN.

  • Care Co-ordinator

Care Co-ordinator works closely with GPs and other primary care professionals and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers in the community.

  • Mental Health practitioner

Help practice by reviewing, triaging and signposting patients as appropriate within Redbridge/NELFT mental health services and other agencies.
